HOW TO SUBMIT

Email the following:.

1. Full name of talent
2. Full name of parent/guardian (if talent is a minor)
3. Contact cell number
4. Email address
5. Up to 3 photos (they do not need to be professional photos)
6. Talents DOB (date of birth)
7. Any special skills
8. Resume if applicable
9. Measurements: height, weight, chest, waist, hips and inseam
10. What City and State do you live in?
